Adult Cancer Incidence
According to the American Cancer Society, cancer affects one in three Americans in their lifetime. We aimed to trend how cancer screenings and diagnoses have changed over time, and how they were influenced by the pandemic. Our data tracker shows the quarterly rates of overall and advanced cancer diagnoses over the past five years to understand overall trends in cancer, including breakdowns for specific cancer types.
Data
Cancer continues to be a leading cause of death worldwide. Many initiatives have sought to reduce the overall incidence of cancer and especially focus on catching cancer at earlier stages, when it is more treatable and less likely to lead to death. This data tracker focuses on the overall incidence of all types of cancers and the rate of initial diagnosis at advanced stages.

Metric Definitions
- Numerator
- The number of distinct patients identified in that quarter having the event of interest (Cancer Diagnosis, Cancer Screening, Pre-Cancer Diagnosis, or Advanced Cancer). Patients are excluded if they have previously had a Cancer Diagnosis of that type. Departments must contribute data for the entire study period to be included.
- Denominator
- The number of distinct patients identified in that quarter having an in-person encounter. Departments must contribute data for the entire study period to be included.
- Cancer Diagnosis
- A diagnosis with ICD-10-CM code of C00*, C01*, C02*, C03*, C04*, C05*, C06*, C07*, C08*, C09*, C10*, C11*, C12*, C13*, C14*, C15*, C16*, C17*, C18*, C19*, C20*, C21*, C22*, C23*, C24*, C25*, C26*, C30*, C31*, C32*, C33*, C34*, C37*, C38*, C39*, C40*, C41*, C4A*, C43*, C44*, C45*, C46*, C47*, C48*, C49*, C50*, C51*, C52*, C53*, C54*, C55*, C56*, C57*, C58*, C60*, C61*, C62*, C63*, C64*, C65*, C66*, C67*, C68*, C69*, C70*, C71*, C72*, C73*, C74*, C75*, C76*, C80*, C81*, C82*, C83*, C84*, C85*, C86*, C88*, C89*, C90*, C91*, C92*, C93*, C94*, C95*, C96*, C7A*.

- Advanced cancer cases are defined as either a Metastatic Diagnosis within 3 months after an initial Cancer Diagnosis, or the cancer classified as Stage 4 within 3 months after an initial Cancer Diagnosis of the same type.
- Metastatic Diagnosis
- A diagnosis with ICD-10-CM code of C7B*, C78*, C79*, C77*.

Routine cancer screenings are a primary preventative care measure that decreased dramatically at the start of the pandemic with 94% decreases in both breast cancer screenings and cervical cancer screenings and an 86% decrease in colon cancer screenings. While cancer screening rates rebounded in 2021 from early pandemic lows, they were still lower than expected.

In May 2020, we reported a significant drop in the weekly rate of routine cancer screenings. At that time, there was a 94% decrease in breast cancer screenings, an 86% decrease in colon cancer screenings, and a 94% decrease in cervical cancer screenings. In July 2020, our follow-up study showed weekly screening rates for these three cancers were on the rise but had not yet reached pre-pandemic levels, remaining 29%-36% lower than their pre-COVID-19 levels.

ExpandDelayed Cancer Screenings
EHR data show a recent abrupt drop – between 86% and 94% – in preventive cancer screenings performed across the United States, presumably due to access disruptions caused by COVID-19. The data set includes 2.7 million patient records from 39 organizations that represent 190 hospitals spanning 23 states.
